Zusammenfassung

Das Mediastinum bezeichnet den thorakalen Raum, der ventral durch das Sternum, dorsal durch die Wirbelsäule und seitlich durch die Pleura mediastinalis gebildet wird. Eine mediastinale Raumforderung stellt oftmals einen Zufallsbefund dar. Das breite Spektrum der zugrunde liegenden Erkrankungen und die anatomische Nähe zu zentralen Strukturen sind die besonderen Herausforderungen in der Diagnostik und Behandlung. Die Kombination aus Anamnese, körperlicher Untersuchung, Bildgebung und Laboruntersuchungen liefert in vielen Fällen bereits entscheidende Hinweise auf die Entität des Tumors. Neben teils erforderlichen diagnostischen Eingriffen zur histologischen Sicherung eines unklaren mediastinalen Tumors ist die radikale Resektion insbesondere bei Tumoren des Thymus der wichtigste prognostische Faktor im Hinblick auf die Prognose der Patienten. In ausgewiesenen Zentren können auch fortgeschrittene Tumoren mit Beteiligung benachbarter Strukturen multimodal behandelt werden.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
