Endoskopische Techniken bei Frühkarzinomen im oberen und unteren Gastrointestinaltrakt

Zusammenfassung

Hintergrund

Die endoskopische Resektion (ER) stellt eine minimalinvasive Therapieoption zur Behandlung gastrointestinaler Frühkarzinome dar.

Fragestellung

Darstellung aktueller Therapieempfehlungen und publizierter Ergebnisse der ER beim Ösophagusfrühkarzinom, beim Magenfrühkarzinom und beim kolorektalen Frühkarzinom.

Material und Methoden

Auswertung der aktuellen Literatur. Erläuterung anhand von endoskopischen Fallbeispielen.

Ergebnisse

Neue Technologien wie „narrow-band imaging“ (NBI) haben die endoskopische Diagnostik und Dignitätsbeurteilung gastrointestinaler Frühneoplasien verbessert. Die Entwicklung der endoskopischen Submukosadissektion (ESD) hat durch eine verbesserte R0-Resektionsrate und ein vermindertes Rezidivrisiko die Rate kurativer ER erhöht. Resektionskriterien für gastrointestinale Frühkarzinome sind in nationalen und internationalen Leitlinien festgelegt. Beim Magenfrühkarzinom unterscheiden sich deutsche („guideline criteria“) und asiatische Leitlinien („expanded criteria“). Neue Daten aus Deutschland zeigen keinen Überlebensunterschied nach ESD von Magenfrühkarzinomen der „guideline criteria“ und der „expanded criteria“. Für kolorektale Frühkarzinome gilt aktuell eine Submukosainvasion von 1000 µm als Grenze der ER. Möglicherweise kann diese Grenze für bestimmte Subgruppen ohne weitere Risikofaktoren erweitert werden; weitere Daten hierzu sind abzuwarten.

Schlussfolgerungen

Es gibt erhebliche Fortschritte in der endoskopischer Diagnostik und Therapie. Erste europäische Studien können die asiatischen Daten bestätigen. Weitere Studien aus der westlichen Welt sind erforderlich, um die Möglichkeiten und Grenzen der endoskopischen Resektion durch verbesserte Datenlage noch präziser zu definieren.

Bei Immuntherapien das erhöhte Thromboserisiko beachten

Unter modernen Systemtherapien versechsfacht sich das VTE-Risiko. Warum diese Daten relevant für die Behandlung krebsassoziierter Thrombosen sind, erläutert Prof. F. Langer im Interview.
