Stellenwert der MRT/Ultraschall-Fusionsbiopsie für die Detektion und Verlaufskontrolle des Prostatakarzinoms

Zusammenfassung

Der multiparametrischen Magnetresonanztomographie (mpMRT) der Prostata kommt eine wachsende Bedeutung für die Primärdiagnostik und die Verlaufskontrolle des Prostatakarzinoms (PCa) im Rahmen der „active surveillance“ zu. Aktuelle Studien belegen, dass die Verwendung der mpMRT die Detektionsrate klinisch signifikanter PCa (csPCa) erhöhen sowie die Detektionsrate klinisch insignifikanter PCa und die Anzahl unnötiger Biopsien reduzieren kann. Die Information des mpMRT kann mittels In-bore-Biopsie, kognitiv bzw. durch Fusionsbiopsiesysteme genutzt werden. Es existieren keine eindeutigen Empfehlungen zum Zugangsweg bei der Fusionsbiopsie (transrektal oder perineal), wobei für den transrektalen Zugang höhere Raten an rektalen Blutungen und Infektionen gegenüber dem perinealen Zugang beschrieben sind. Die zunehmende Rate an Antibiotikaresistenzen sowie die Indikationseinschränkungen für Chinolone stellen eine Herausforderung für die transrektale Biopsie dar.
