Mammakarzinom in der Schwangerschaft: Systemtherapie

Zusammenfassung

Die Therapie des Mammakarzinoms in der Schwangerschaft stellt eine interdisziplinäre Herausforderung dar, die nahezu immer einen Konflikt zwischen der optimalen Therapie für die Mutter auf der einen Seite, die mitunter auch die Terminierung der Schwangerschaft bedeuten kann, und dem Wohl des Kindes auf der anderen Seite bedeutet. Eine insuffiziente Therapie allerdings, die vorgibt, den Feten nicht zu schädigen, kann die Gesundheit der Mutter gefährden und möglicherweise von einer kurativen in eine palliative Situation führen. Daher sollte das Therapieziel innerhalb wie außerhalb der Schwangerschaft dasselbe sein: Gewährleistung der lokoregionären Kontrolle und Vermeidung einer Fernmetastasierung. Eine Systemtherapie sollte nicht innerhalb des ersten Trimenons erfolgen, da die Organogenese in diesen Zeitraum fällt. Während eine anthrazyklinhaltige Chemotherapie im zweiten Trimenon relativ sicher durchgeführt werden kann, ist eine taxanhaltige Chemotherapie, eine Tamoxifen- oder Strahlentherapie erst nach der Entbindung durchzuführen. Im Zuge aller therapeutischen Interventionen bleibt die prinzipielle Sorge, welche Langzeitfolgen daraus für das Neugeborene entstehen.
