Endokrine Kombinationstherapien beim lokal fortgeschrittenen, metastasierten Mammakarzinom – zusammen besser als allein

Zusammenfassung

In den letzten Jahren hat sich die endokrine Therapie des hormonrezeptorpositiven, lokal fortgeschrittenen/metastasierten Mammakarzinoms erfolgreich weiterentwickelt. Neue Substanzen wurden entwickelt, um endokrine Resistenzen zu überwinden, altbekannte Medikamente mit neuen Substanzen kombiniert. Die Gruppe der CDK4/6(„cyclin-dependent kinases“)-Inhibitoren hat inzwischen seine Überlegenheit gegenüber der endokrinen Monotherapie gezeigt. Aber auch neue Substanzen, wie der PI3K(„phosphoinositide 3‑kinase inhibitor“)-Inhibitor Alpelisib, bieten eine Therapieoption für Patientinnen mit vorliegender PIK3CA(„phosphatidylinositol‑4,5‑bisphosphate 3‑kinase catalytic subunit α“)-Mutation. Die Kombination von Everolimus, einem mTOR(„mechanistic target of rapamycin“)-Inhibitor, mit Exemestan behält auch weiterhin einen Stellenwert in der Therapie des metastasierten Mammakarzinoms. Endokrine Kombinationstherapien sind aus der Praxis nicht mehr wegzudenken und haben zu einer deutlichen Prognoseverbesserung und teilweise nachgewiesen verlängertem Gesamtüberleben geführt. In Studien müssen offene Fragen zur Therapiefolge und -kombinationen geklärt werden, und die Mechanismen der endokrinen Resistenz müssen weiter wissenschaftlich untersucht werden, um diese zu umgehen. Dafür könnten künftig sogar Triplet-Therapiekombinationen infrage kommen.

Bei Immuntherapien das erhöhte Thromboserisiko beachten

Unter modernen Systemtherapien versechsfacht sich das VTE-Risiko. Warum diese Daten relevant für die Behandlung krebsassoziierter Thrombosen sind, erläutert Prof. F. Langer im Interview. So kann es durch Immuntherapien zu inflammatorischen Syndromen z.B. im GI-Trakt kommen. Nebenwirkungen wie Durchfall oder Mukositis haben dann Einfluss auf die Wirksamkeit oraler Antikoagulantien. Aber auch in punkto Blutungsrisiko ist Vorsicht geboten. Wann hier bevorzugt NMH eingesetzt werden sollten, erläutert Prof. Langer im Interview.

CAT-Management ist ganz einfach – oder doch nicht?

Krebsassoziierte venöse Thromboembolien (CAT) haben in den vergangenen Jahren stetig zugenommen. Was hat der Anstieg mit modernen Antitumortherapien zu tun? Finale OS-Analyse der MONARCH-3-Studie vorgestellt
