Zusammenfassung

Gallengangsverletzungen werden nach abdominellem Trauma, postoperativ nach Cholezystektomie, Leberresektion oder -transplantation oder als Komplikation nach endoskopischer retrograder Cholangiopankreatikographie (ERCP) beobachtet. Der klinische Befund zeigt in Abhängigkeit vom Ausmaß der Verletzung erhebliche Variationen und reicht vom asymptomatischen Zufallsbefund bis zur lebensbedrohlichen Sepsis. In vielen Fällen können extra- und intrahepatische Läsionen mit den etablierten endoskopischen Methoden erfolgreich behandelt werden. Temporäre Metallstents gewinnen bei ausgedehnten Läsionen oder Misserfolg der Standardtherapie zunehmend an Bedeutung. Bei einem ausgedehnten Trauma oder in den wenigen Fällen einer substanziellen Schädigung, die interventionell nicht zu erreichen oder zu kontrollieren ist, bedarf es der chirurgischen Revision.
