Abstract
Background
Craniofacial duplication, or diprosopus, is a rare congenital malformation characterized by the partial or complete duplication of craniofacial structures, including the mandible, maxilla, oral cavity, and associated soft tissues. The condition is believed to result from abnormal embryological development and has a higher incidence among females. Due to its rarity and varied clinical presentations, diagnosis can often be delayed or mistaken for more common pathologies.
Purpose
This case report aims to describe the clinical presentation, diagnostic challenges, and surgical management of a rare case of partial duplication of the left hemimandible in a 7-year-old female, initially misdiagnosed in early childhood. The report also highlights the importance of imaging and surgical intervention in the management of such craniofacial anomalies.
Results
The patient presented with a bony hard swelling in the left lower jaw, with no intraoral findings or functional impairment. Initial diagnosis in infancy misidentified the lesion as a congenital cyst. Imaging, including CBCT and OPG, later revealed a duplicated segment of the left hemimandible containing developing tooth buds and an accessory neurovascular bundle. Surgical excision of the duplicated bony segment was performed via a submandibular approach under general anesthesia. The procedure was uneventful, with preservation of neurovascular structures and satisfactory postoperative recovery.
Conclusions
Partial duplication of the hemimandible poses diagnostic and surgical challenges due to its rarity and similarity to other craniofacial conditions. Early use of radiographic imaging and careful surgical planning are critical for accurate diagnosis and successful management. Maxillofacial surgeons play a key role in ensuring functional and aesthetic outcomes in such complex craniofacial anomalies.
