Zusammenfassung

Die Ergometrie gehört zur Basisdiagnostik in der Kardiologie. Im Lichte der enormen Fortschritte der kardialen Bildgebung und Risikostratifikation ist eine Neubewertung dieses Untersuchungsverfahrens sinnvoll. In einigen Fragestellungen sind die Empfehlungen zur Anwendung der Ergometrie präzisiert worden. Insbesondere bei der Diagnosestellung der koronaren Herzerkrankung (KHK) hat sich eine Änderung dahingehend ergeben, dass die Anwendung der Ergometrie in der primären Diagnostik in Abhängigkeit von der Vortestwahrscheinlichkeit für das Vorliegen einer KHK abhängig gemacht wird. Liegt diese bei 15–65 %, so ist nach den aktuellen ESC-Leitlinien zum Management der stabilen KHK eine Ergometrie sinnvoll. Weitere Änderungen zu den DGK-Leitlinien von 2000 bestehen beispielsweise in der Übernahme der Empfehlungen der American Heart Association (AHA) hinsichtlich Durchführung der Ergometrie durch geschultes nichtärztliches Personal sowie der Arztanwesenheit während der Prozedur.
