Zusammenfassung

Das Marfan-Syndrom ist mit einer Prävalenz von etwa 1–2:10000 die häufigste hereditäre Bindegewebserkrankung mit vaskulärer Komponente. Ursache sind Mutationen in dem Gen, das für das extrazelluläre Matrixprotein Fibrillin‑1 kodiert. Die Erkrankung wird autosomal-dominant vererbt. Es handelt sich um eine Multisystemerkrankung mit Beteiligung von Aorta und Mitralklappe sowie Augen- und Skelettveränderungen. Primäre Ziele in der Betreuung von Patienten mit Marfan-Syndrom sind die korrekte Diagnosestellung und damit die Verhinderung eines Dissektionsereignisses. Als diagnostisches Verfahren der 1. Wahl kommt die transthorakale Echokardiographie zum Einsatz. Zur genauen Darstellung der Aorta wird im weiteren Verlauf eine bildgebende Untersuchung mithilfe der MRT oder der CT durchgeführt. Im vorliegenden ersten von 2 Teilen wird detailliert auf die Genetik, Diagnose, Differenzialdiagnose, die bildgebende diagnostische Untersuchung, Follow-up, die kardiovaskulären Probleme sowie die Manifestation an Aorta und den großen Gefäßen bei Patienten mit Marfan-Syndrom eingegangen. Eine strukturierte Langzeitbetreuung ist für Patienten mit Marfan-Syndrom essenziell.
