Medikamentöse Therapie der amyotrophen Lateralsklerose: ein Update

Die amyotrophe Lateralsklerose (ALS) ist eine nicht heilbare, rasch fortschreitende neurodegenerative Erkrankung. Im Verlauf der Erkrankung kommt es zu einem progredienten Untergang des oberen und unteren Motoneurons, pathologisches Korrelat sind Ablagerungen fehlgefalteter Proteine im Zytoplasma (>90 % fehlgefaltetes und hyperphosphoryliertes transactive response DNA binding protein 43 [TDP43]). Klinisch stehen zunehmende Paresen und Atrophien der Extremitätenmuskulatur, Dysphagie und Dysarthrie und schließlich eine Einschränkung der Atemmuskulatur im Vordergrund. In > 50 % der Patienten kommt es jedoch auch zu messbaren neuropsychologischen Auffälligkeiten bis hin zur Entwicklung einer frontotemporalen Demenz (ALS-FTD). Das mediane Überleben von Erkrankungsbeginn bis zum Tod der Patienten beträgt 2–4 Jahre, die Prognose der ALS ist dabei insbesondere von der im Verlauf auftretenden Lähmung der Atemmuskulatur abhängig. Die ALS zählt mit einer Inzidenz von 2–3/100.000 in Europa zu den seltenen Erkrankungen. Sie tritt am häufigsten zwischen dem 60. und 70. Lebensjahr auf, Männer sind etwas mehr betroffen als Frauen. Der Großteil der Erkrankten leidet unter einer sporadischen Form (>90 %), bei ca. 10 % der Patienten liegt eine familiäre ALS mit positiver Familienanamnese vor. Bei der familiären ALS konnten bereits zahlreiche pathogene Genmutationen nachgewiesen werden, in Mitteleuropa sind die am häufigsten betroffenen Gene C9ORF72 (ca. 30 %), Superoxide Dismutase-1 (SOD‑1) (ca. 10 %), Fused in Sarcoma (FUS) (ca. 5 %, insbesondere juvenile Formen) und TDP-43 (ca. 4 %; für weitere Details [1]). …
