Medikamentöse Therapie der Analfissur

Zusammenfassung

Die medikamentöse Behandlung hat sich in den letzten Jahren als Erstlinientherapie der chronischen Analfissur durchgesetzt. Topisch angewandte Kalziumantagonisten und Nitrate führen zu einer Reduktion des analen Sphinkterdrucks, einer Verbesserung der Durchblutung und mit guter Evidenz zu einer gegenüber Placebo höheren Heilungsrate von 60–70 %. Die Heilungsraten von Kalziumantagonisten und Nitropräparaten unterscheiden sich nicht signifikant. Nitropräparate haben eine höhere Nebenwirkungsrate (v. a. Kopfschmerzen), es reicht eine Konzentration von 0,2 % Glyceroltrinitrat (GTN) 3‑mal täglich aus. Kalziumantagonisten sind dagegen in Deutschland nicht für die Therapie der Analfissur zugelassen („off-label use“). Es gibt eine NRF-Rezeptur (neues Rezeptur-Formularium) für eine 2 %ige Diltiazemcreme, die 2‑mal täglich aufgetragen wird. Die Therapie wird 6 bis 8 Wochen bis zu der unbedingt erforderlichen Kontrolluntersuchung durchgeführt, bei einem Teilerfolg ggf. auch länger. Zusätzlich sind eine Stuhlgangregulierung (i. d. R. Flohsamenschalen) und ein richtiges Defäkationsverhalten wichtig, die als Rezidivprophylaxe unbedingt beibehalten werden sollten. Bei Erfolglosigkeit sollte eine andere Therapie (z. B. Fissurektomie nach Gabriel, alternativ Botulinumtoxin) erfolgen. Bei einer (subanodermalen) Fistel oder ausgeprägten Sekundärveränderungen ist an eine primäre Operation ohne medikamentöse Vorbehandlung zu denken.
