Aktuelle therapeutische Ansätze bei ZNS-Tumoren

Zusammenfassung

Hintergrund

Pädiatrische Hirntumoren umfassen biologisch, klinisch und prognostisch ein äußerst heterogenes Spektrum von Entitäten.

Methoden

Aktuelle diagnostische und therapeutische Entwicklungen werden dargestellt.

Ergebnisse

Integrierte Analysen erlauben die Einteilung für viele Entitäten in molekulare Subgruppen mit unterschiedlichen klinischen und biologischen Charakteristika. Für niedriggradige Gliome ist eine adjuvante Therapie v. a. bei nichtoperablen symptomatischen oder progredienten Tumoren indiziert. In der zukünftigen LOGGIC-Studie (Low Grade Glioma in Children) erfolgt ein randomisierter Vergleich von Carboplatin/Vincristin, Trametinib und Vinblastin. Für hochgradige Gliome besteht die Standardtherapie bei älteren Kindern aus einer fokalen Bestrahlung und Temozolomid, experimentelle Ansätze werden u. a. in der Studie HIT-HGG 2013 evaluiert. Für Medulloblastome wird in der aktuellen SIOP-PNET5-MB-Studie bei Wnt-Aktivierung die Reduktion der kraniospinalen Bestrahlungsdosis und der Chemotherapie sowie bei Standardrisiko die Intensivierung durch Carboplatin evaluiert. Bei jungen Kindern mit SHH-aktiviertem Medulloblastom ist eine intraventrikuläre Chemotherapie mit Methotrexat in Kombination mit systemischer Chemotherapie wirksam. Für Ependymome wird aktuell u. a. der Effekt einer Erhaltungschemotherapie nach kompletter Resektion und Strahlentherapie evaluiert (SIOP-Ependymoma II). Für Kinder mit atypischen teratoiden/rhabdoiden Tumoren (AT/RT) besteht die postoperative Therapie nach EU-RHAB-Register (European Rhabdoid Registry) aus intensiver Chemotherapie und Bestrahlung. Konzepte für weitere Entitäten und neue strahlentherapeutische Entwicklungen werden beschrieben.

Schlussfolgerung

Wirkprofile und Spätfolgen der multimodalen Behandlungsstrategien müssen für die verschiedenen Entitäten gezielt evaluiert und optimiert werden.

Umfrage: Topika mit Dexpanthenol bei Radiodermatitis empfohlen

In der topischen Prävention der akuten Radiodermatitis werden Zubereitungen mit Dexpanthenol oder Harnstoff von deutschsprachigen Fachkreisen in der Radioonkologie am häufigsten empfohlen und als am wirksamsten bewertet. Bei der Behandlung der strahlenbedingten Hautschäden liegen Topika mit Dexpanthenol oder Kortikosteroide vorn [1].

Handekzem: Adhärenz bei topischer Therapie nicht zufriedenstellend

Einer klinischen Studie zufolge wendet nur etwa die Hälfte der Ekzem-Patient:innen ihre topische Therapie mit Kortikosteroiden wie verordnet an. Darüber hinaus nahm die Adhärenz im Zeitverlauf weiter ab. Bei einer gleichzeitig applizierten barrierestabilisierenden Basiscreme blieb die Anwendungsfrequenz dagegen über die Zeit stabil [1].

Finale OS-Analyse der MONARCH-3-Studie vorgestellt
