Polyzystisches Ovarsyndrom – praktische Umsetzung der internationalen Leitlinie

Zusammenfassung

Im Juli 2018 wurde eine internationale Leitlinie zum PCOS (polyzystisches Ovarsyndrom) publiziert. Welche Neuerungen hat diese ergeben? Zunächst bestätigte die Leitlinie erneut die Rotterdam-Kriterien zur Diagnosestellung. Das PCOS ist weiterhin eine Ausschlussdiagnose. Das Anti-Müller-Hormon wurde noch nicht als Diagnosekriterium aufgenommen. Es wird empfohlen, bereits ab Diagnosestellung präventiv Lifestyle-Interventionen zur Vermeidung von Übergewicht/Adipositas anzuregen und auch ein Screening/Management assoziierter depressiver Symptome/Angststörungen anzubieten. Letzteres hat sich in der Praxis noch nicht durchgesetzt. Kombinierte hormonelle Kontrazeptiva werden weiterhin als pharmakologische First-line-Therapie der Androgenisierungserscheinungen und Zyklusunregelmäßigkeiten angesehen. Zusätzlich zu Lifestyle-Interventionen kann Metformin zur Therapie metabolischer Probleme eingesetzt werden. Bei Kinderwunsch stellt Letrozol nun die First-line-Therapie zur Ovulationsinduktion dar. Inositol und operative Maßnahmen (bariatrische Chirurgie) sind weiterhin experimentelle Therapieansätze.
