Methocarbamol bei akuten Rückenschmerzen

Eine randomisierte, doppelblinde, placebokontrollierte Studie

Zusammenfassung

Hintergrund:

Muskelrelaxantien sind ein wichtiger Teil der Behandlungsstrategie bei Rückenschmerzen (LBP, Low Back Pain), einem der häufigsten gesundheitlichen Probleme in Industrienationen. Für diese Indikation hat die Europäische Arzneimittelbehörde (EMA) jüngst Anwendungsausschlüsse für Muskelrelaxantien, aber auch Antiphlogistika und Analgetika, und sogar Marktrücknahmen (Tetrazepam) angeordnet. Als einzig explizit zugelassenes Muskelrelaxans ist Methocarbamol verblieben. Die Anwendung des Wirkstoffs bei LBP mit myofaszialer Komponente ist breit etabliert, obwohl bisher noch keine klinisch relevanten Studien publiziert wurden. Daher werden in dieser Arbeit bislang noch nicht publizierte Daten aus dem Jahr 2002, bei denen der Schwerpunkt auf der Wirksamkeit von Methocarbamol lag, zusammengefasst und erneut post hoc beurteilt.

Methode:

Es handelte sich um eine randomisierte, placebokontrollierte Multicenterstudie. Eingeschlossen waren Patienten, die seit mindestens 24 Stunden unter akuten Rückenschmerzen, Schmerzen im Becken-/Lendenbereich und eingeschränkter Beweglichkeit litten. Die Patienten erhielten entweder orales Methocarbamol (Ortoton®) (n = 98) oder Placebo (n = 104). Die Behandlung dauerte maximal 8 Tage, konnte allerdings individuell beendet werden, sobald ein schmerzfreier Zustand erreicht war. Die individuelle Schmerzwahrnehmung wurde mit Hilfe einer visuellen Analogskala (VAS) quantifiziert. Die Beweglichkeitseinschränkung wurde anhand des Finger-Boden-Abstands und des Schober-Maßes bestimmt. Zudem beurteilten Patienten und Prüfärzte die Wirksamkeit der Behandlung anhand einer Bewertungsskala.

Ergebnisse und Schlussfolgerungen:

In der Verumgruppe beendeten 44% der Patienten die Behandlung vorzeitig, weil sie schmerzfrei waren (Placebo: 18%) und 19% aufgrund mangelnder Wirksamkeit (Placebo: 52%; p<0,0001). Die Werte für die Beweglichkeit (Finger-Boden-Abstand, Schober-Maß) und die von Prüfarzt und Patient am individuellen Studienende eingeschätzte Verbesserung der Beweglichkeit fielen zugunsten der mit Metho- carbamol behandelten Patienten aus. Zum Abschlusstermin beurteilten in der Methocarbamolgruppe 67% der Patienten (Placebo: 35%) und 70% der Prüf ärzte (Kontrolle: 35%) die Behandlung als wirksam. Es wurden keine schwerwiegenden Nebenwirkungen beobachtet (7 Symptome bei 5 Patienten).

Schlussfolgerung:

Oral verabreichtes Methocarbamol stellt eine wirksame und gut verträgliche therapeutische Option für Patienten mit akutem LBP und den typischerweise damit assoziierten Beweglichkeitseinschränkungen dar.
