A 40-year-old woman without any known genetic predisposing disease presented to the outpatient consultation clinic with complaints of memory loss without any daily living limitations. Her past medical history was remarkable for central nervous system conventional radiotherapy treatment at a dosage of 50 Gy for craniopharyngioma 35 years before (at the age of five). She was also receiving levetiracetam for symptomatic epilepsy which was well controlled. At examination, she scored 25 out of 30 on the Montreal Cognitive Assessment test, presented slight microcrania and moderate alopecia. The rest of the neurological examination was unremarkable. The brain magnetic resonance imaging revealed the presence of avidly contrast enhancing mirror convexity meningiomas located near the supramarginal gyrus (Fig. 1a). Previous specific and detailed radiotherapy plan of the patient was not available, but the lesions have developed along the radiation trajectory (Fig. 1b). Contrary to radiation-induced meningioma which are a frequent and well-described complication of radiotherapy [1, 2], the occurrence of mirror meningioma is seldom reported [3, 4]. Other locations of mirror meningiomas reported in the literature are the sphenoid bone, optic nerves, parasagittal, falx or foramen magnum [4]. The causal association between the childhood radiotherapy and the late occurrence of the meningiomas here described is reinforced by the coherence of the tumor location and the entrance fields of the radiotherapy treatment for craniopharyngioma.
