Desmoidfibromatose: ein Fallbericht

Zusammenfassung

Wir berichten über den Fall eines 57-jährigen Patienten mit riesiger, retrovesikal gelegener Desmoidfibromatose, welche im Rahmen des präoperativen Stagings vor Prostatektomie zufällig detektiert wurde. Es handelt sich dabei um seltene, semimaligne Weichteiltumoren aus monoklonalen Fibroblasten und Kollagenfasern, deren Ätiologie weitgehend ungeklärt ist. Ihr Auftreten ist meist sporadisch, kann in einigen Fällen jedoch mit der der familiären adenomatösen Polyposis (FAP) oder dem Gardner-Syndrom assoziiert sein. Sie wachsen langsam, lokal infiltrativ-destruierend, metastasieren jedoch nicht. Die Therapie der Wahl ist ihre radikale Resektion, die Rezidivrate äußert hoch. Eine postoperative Radiatio wird in der Literatur kontrovers diskutiert. Anhand des Fallbeispiels werden pathogenetische, klinische und therapeutische Aspekte dieser seltenen Erkrankung diskutiert.
