Mukormykosen bei Patienten mit Diabetes mellitus

Zusammenfassung

Hintergrund

Mukormykosen werden durch Schimmelpilze der Ordnungen Mucorales und Entomophthorales verursacht. Die Sporen kommen ubiquitär vor und werden durch Inhalation, Ingestion oder traumatische Inokulation übertragen. Zu Infektionen kommt es meist bei Immunsupprimierten. Eine besondere Risikogruppe sind Patienten mit Diabetes mellitus. Mukormykosen können in diesem Kollektiv kutan, gastrointestinal, pulmonal, am häufigsten aber sinuorbitozerebral auftreten. Letztere Form schreitet rasch fort und führt über den plötzlichen einseitigen Verlust des Auges aufgrund verspäteter Diagnose oft zum Tod.

Ziel der Arbeit

Es wird ein Überblick über die Diagnostik und Therapie der Mukormykose bei Patienten mit Diabetes mellitus gegeben.

Material und Methoden

Für die Erstellung dieses Beitrags wurden die europäische Leitlinie sowie ausgewählte aktuelle Literatur zum Thema aufgearbeitet.

Ergebnisse

Die Mukormykose ist eine äußerst seltene Erkrankung, die mit einer hohen Sterblichkeit assoziiert ist. Das aggressive, invasive Pilzwachstum überschreitet regelhaft Organgrenzen. Medikament der Wahl ist in der Erstlinientherapie liposomales Amphotericin B. Bei schlechtem Ansprechen sollte eine Umstellung auf Posaconazol erwogen werden. Eine Kombinationstherapie der beiden Medikamente stellt eine weitere Möglichkeit dar. Darüber hinaus steht seit 2016 Isavuconazol als gut verträgliche Alternative zur Verfügung.

Diskussion

Laufende klinische Studien widmen sich derzeit der Entwicklung neuer, antimykotisch wirksamer Substanzen sowie dem neuen Ansatz der Immuntherapie. Weiterhin wird die Steigerung der Effektivität bei der Kombination verschiedener Therapeutika untersucht.
