Zusammenfassung

Multiple Sklerose und Neuromyelitis-optica-Spektrum-Erkrankungen sind autoimmun-entzündliche Erkrankungen des zentralen Nervensystems, die zu vielfältigen neurologischen Beschwerden und Schmerzsyndromen führen können, sowohl akut im Rahmen eines Erkrankungsschubs als auch chronisch, wobei Letzteres die Lebensqualität teils erheblich reduziert. Die Ätiologie der Schmerzen ist sehr heterogen, deren rasche differenzialdiagnostische Zuordnung jedoch entscheidend, um differenzierte Therapiestrategien einleiten zu können. Von Schmerzen als mögliches (frühes) Symptom eines akuten Erkrankungsschubs müssen chronische Schmerzen abgegrenzt, weiter differenziert und individuell behandelt werden. Hierzu zählen zentral-neuropathische und spastikassoziierte Schmerzen, muskuloskeletale Schmerzen durch Fehlbelastung sowie Schmerzen als Nebenwirkungen der Immuntherapien und im Kontext von Komorbiditäten. Aufgrund der mangelhaften Datenlage sind die Therapiestrategien häufig unzureichend evidenzbasiert.
