Zusammenfassung

Hintergrund

Zunehmend häufig sind Patienten mit multiresistenten bakteriellen Erregern kolonisiert. Dies führt zu einer steigenden Belastung des Gesundheitssystems, da vermehrt erweiterte Hygienemaßnahmen umgesetzt werden müssen. Im Infektionsfall können von multiresistenten Erregern Gefahren für den Patienten ausgehen, wenn die antibiotische Therapie unzureichend greift.

Ziel der Arbeit

Ziel ist eine Risikobetrachtung auf der Basis der relevanten multiresistenten Bakteriengruppen, ihrer Epidemiologie sowie der prädisponierenden Faktoren einer Akquisition, um insbesondere für Patienten mit chronischen Nierenkrankheiten das Gefährdungspotenzial abzuschätzen. Ferner sollen Präventionsstrategien diskutiert werden, um eine Ausbreitung der Multiresistenz insgesamt, aber auch die individuellen Risiken einzudämmen.

Material und Methoden

Selektiver Literaturreview unter Einschluss von PubMed-Recherchen sowie der Analyse von deutschen Leitlinienempfehlungen zum Umgang mit multiresistenten Erregern.

Ergebnisse

Praktisch relevante multiresistente Erreger sind neben methicillinresistenten Staphylococcus aureus (MRSA) vor allem vancomycinresistente Enterokokken (VRE) sowie multiresistente gramnegative Bakterien verschiedener Spezies, die aufgrund der Resistenz gegen 3 (3MRGN) oder 4 (4MRGN) der gebräuchlichen Antibiotikaklassen eingeteilt werden. Ein Risikofaktor für eine Kolonisation ist neben breitem Einsatz von Antibiotika auch die nosokomiale Übertragung. Ein individuelles Gefährdungspotenzial bei Kolonisation ist für MRSA und 4MRGN evident, für VRE und 3MRGN zumindest möglich. Erweiterte Hygienemaßnahmen unter Berücksichtigung der Transmissionswege sind erforderlich.

Diskussion

Die Ausbreitung multiresistenter Keime hat zunehmenden Einfluss auch auf die Organisation der Nierenersatztherapie. Während eine nosokomiale Transmission solcher Erreger in der stationären Versorgung vorkommt und durch erweiterte Hygienemaßnahmen unterbunden werden muss, sind die Risiken in der ambulanten Dialyse geringer. Auch hier ist jedoch ein rationales Vorgehen zur Vermeidung der Verbreitung unverzichtbar.
