Zusammenfassung

Nach der ACCORD-Studie bei Diabetikern liegt nun mit der SPRINT-Studie bei nichtdiabetischen Patienten eine weitere hochklassige Studie zum Effekt einer intensiven Blutdrucksenkung vor. Im Gegensatz zu Diabetikern kann bei Hypertonikern allgemein durch systolische Blutdrucksenkung auf etwa 120 mmHg eine signifikante Reduktion von kardiovaskulären Ereignissen und Todesfällen erreicht werden. Dieser günstige Effekt war tendenziell auch bei CKD(„chronic kidney disease“)-Patienten zu beobachten. Jedoch kommt es bei intensiver Blutdrucksenkung nicht nur häufiger zu Komplikationen wie Hypotensionen, Synkopen und Elektrolytstörungen, sondern auch vermehrt zu Episoden akuter Nierenschädigung („acute kidney injury“, AKI). Zudem scheint sich in SPRINT die Beobachtung aus vorangehenden Studien zu bestätigen, dass eine intensive Blutdrucksenkung mit dem vermehrten Neuauftreten einer CKD bzw. der stärkeren Progression einer CKD verknüpft ist.

SGLT-2-Inhibitoren für alle Patient:innen mit chronischer Herzinsuffizienz empfohlen

Das ESC-Leitlinien-Update 2023 bedeutet einen Paradigmenwechsel in der Behandlung der Herzinsuffizienz (HF), denn nun werden SGLT-2i sowohl für HFrEF, als auch für HFmrEF und HFpEF empfohlen. Dapagliflozin ist nun zur Behandlung aller Patient:innen mit chronischer symptomatischer Herzinsuffizienz zugelassen und bietet somit auch neue Hoffnung für die Therapie von jenen mit HFpEF. In der DELIVER-Studie zeigte der SGLT-2-Inhibitor eine signifikante Reduktion von Herzinsuffizienz-Hospitalisierungen und CV-Todesfällen.
