Zusammenfassung

Zufällig entdeckte Nebennierenveränderungen ohne Krankheitswert sind gegenüber den operativ oder konservativ zu behandelnden Nebennierenerkrankungen abzugrenzen. Vor einer operativen Therapie ist neben den möglichen unterschiedlichen funktionellen Aktivitäten der Tumoren zwischen sporadisch entstandenen singulären und multiplen Nebennierentumoren oder einer knotigen Nebennierenhyperplasie, Nebennierentumoren im Rahmen vererbter Syndromerkrankungen und Metastasen von malignen Tumoren anderen Ursprungs in der Nebenniere zu unterscheiden. Benigne, hormonaktive Tumoren werden minimal-invasiv reseziert, ebenso fraglich maligne Tumoren der Nebennierenrinde. Für große maligne Tumoren mit Infiltration von Umgebungsstrukturen und für Tumoren mit nachgewiesener Lymphknotenmetastasierung ist das primär offene Vorgehen indiziert. Patienten mit Nebennierenerkrankungen sollten immer interdisziplinär beurteilt und bei eindeutiger Operationsindikation ggf. in einem Zentrum mit Erfahrung in der Operation wie auch der postoperativen Betreuung von Nebennierenpatienten behandelt werden.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Prof. Dr. med. Gregor Antoniadis Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
