Schreien und persistierende Unruhe im Säuglings- und Kleinkindalter

Zusammenfassung

Frühkindliche Unruhezustände sind differenziert zu betrachten. Bis zum Ende des 3. Lebensmonats werden sie als „belastendes Symptom“ aufgefasst, das in >85 % der Fälle spontan und folgenlos abklingt. Medikamentöse, alimentäre, chiropraktische und osteopathische Interventionen werden aufgrund mangelnder Wirksamkeitsnachweise nicht empfohlen. Zentral sind die Psychoedukation der Eltern, die Anleitungen zum Handling einschließt, und die Einschätzung psychosozialer Belastungen, einschließlich des Misshandlungsrisikos. Elterliche psychische Gesundheitseinschränkungen sollten erfasst werden. Persistierendes Schreien und Unruhe über den 3. Monat hinaus sind mit späteren psychischen Störungen assoziiert; hier sollte kinderpsychiatrisch-psychotherapeutisch diagnostiziert und therapiert werden. Zur Verbesserung der kindlichen affektiven Selbstregulation ist eine auf die Eltern-Kind-Interaktion ausgerichtete Behandlung indiziert. Elterliche psychische Störungen erfordern spezifische Interventionen.
