A 62-year old woman with a history of alcohol abuse and breast cancer presented to our emergency department 1 day after a mild head trauma. Examination revealed normal consciousness, mydriasis of the right eye, a right-sided hemiparesis (Medical Research Council 4/5, Fig. 1a) and Babinski sign. Computed tomography (CT) of the brain showed an acute subdural haematoma along the right cerebral convexity causing a 5 mm midline shift. Paradoxically, she had no neurologic symptoms related to the right hemisphere.
