Eine 58-jährige Patientin war am Vorstellungsmorgen leblos durch den Ehemann vorgefunden worden. Als Vorerkrankung war ein nichtinsulinpflichtiger Diabetes mellitus bekannt. Nach Verständigung des Rettungsdienstes begann der Ehemann die Laienreanimation bis zum Eintreffen des Rettungsdienstes 8 min später. Bei Asystolie veranlasste der Rettungsdienst die leitliniengerechte Gabe von Adrenalin und setzte die Reanimation fort, nach kumulativ 10 min konnte Kammerflimmern (Abb. 1a) nachgewiesen werden, das durch 2‑malige Defibrillation in den Sinusrhythmus mit Wiederherstellung eines Spontankreislaufs („return of spontaneous circulation“, ROSC) überführt werden konnte. In der kontinuierlichen elektrokardiographischen (EKG) Ableitung zeigten sich jetzt deszendierende ST-Strecken-Hebungen über den Brustwandableitungen V4 und V5 (Abb. 1b). Nach 27 min bildeten sich die ST-Strecken-Hebungen spontan zurück (Abb. 1c). Es erfolgten das präklinische „loading“ mit 250 mg Azetylsalizylsäure und 5000 IE Heparin i.v. sowie die Verbringung der Patientin bei Verdacht auf ST-Strecken-Hebungsinfarkt in das Herzkatheterlabor der Klinik.
