10 Kernaussagen zum Embolic Stroke of Undetermined Source und kryptogenen Schlaganfall

Zusammenfassung

Der kryptogene Schlaganfall ist ein Hirninfarkt, bei dem sich keine Quelle für kardioembolische Ereignisse, keine Mikroangiopathie mit lakunären Infarkten und keine Makroangiopathie mit hochgradiger Stenose der hirnversorgenden Arterien nachweisen lässt. Der kryptogene Schlaganfall ist allerdings nicht operational definiert. Das neue Konzept des Embolic Stroke of Undetermined Source (ESUS) ist definiert als ein nichtlakunärer Schlaganfall in der zerebralen Bildgebung bei Ausschluss einer signifikanten Stenose der hirnversorgenden Arterien durch angiographische Methoden oder Ultraschalldiagnostik. Kardiale Emboliequellen müssen durch EKG-Monitoring und Echokardiographie ausgeschlossen werden. Im Moment erfolgt die Sekundärprävention bei Patienten mit ESUS durch Azetylsalizylsäure. Die Frage ob, nicht-Vitamin-K-orale Antikoagulanzien (NOAK) bei diesen Patienten in der Sekundärprävention wirksam sind, wird derzeit in randomisierten Studien untersucht. Die akute Behandlung des kryptogenen Schlaganfalls/ESUS unterscheidet sich nicht von anderen Schlaganfallsubtypen, da die Schlaganfallätiologie in der Akutphase häufig noch nicht bekannt ist, sondern erst im Lauf der Behandlung auf der Stroke-Unit geklärt wird.
