Neurophysiologisches Monitoring bei operativen Eingriffen

Zusammenfassung

Der Einsatz des intraoperativen neurophysiologischen Monitorings (IONM) gewinnt zunehmend an Bedeutung in der klinischen Routine. Mithilfe des IONM kann neben der Lokalisierung nervaler Strukturen der Funktions- und Versorgungszustand des Nervensystems beim anästhesierten Patienten dargestellt werden. Dies ermöglicht dem Operateur, nervale Strukturen und Hirnareale zu erkennen und, wenn möglich, zu schonen. Dem Anästhesisten erlaubt IONM, sowohl den Operateur auf eine Gefährdung des Patienten hinzuweisen als auch die Narkose und kardiopulmonale Therapie individuell patientengerecht anzupassen. Diese Maßnahmen können u. a. wahrscheinlich die Auftretenshäufigkeit des postoperativen Delirs oder des kognitiven Defizits senken. Um dieses Potenzial auszuschöpfen, müssen Anästhesist und Chirurg die IONM-Verfahren sicher anwenden und deren Ergebnisse interpretieren können. Hierfür sind Kenntnisse über die grundlegenden Techniken und deren Anwendungsmöglichkeiten notwendig. Wichtig ist aber auch das Wissen, inwieweit die Ergebnisse durch z. B. Anästhetika beeinflusst werden und damit die Aussagekraft des IONM beschränken. Die totale intravenöse Anästhesie (TIVA) ist das Anästhesieverfahren, das ein IONM prinzipiell am wenigsten beeinflusst. Wichtig ist die Vermeidung von Bolusgaben, um eine möglichst konstante Anästhetikakonzentration zu gewährleisten. Ebenso sollte auf den Einsatz lang wirksamer Muskelrelaxanzien verzichtet werden, da eine Muskelrelaxierung die Ableitung der Elektromyographiesignale und der Signale motorisch evozierter Potenziale verhindern kann. Durch den Einsatz des IONM können Anästhesist und Chirurg während der intraoperativen Phase Veränderungen der Funktion des Nervensystems erkennen, bevor es zum irreversiblen Schaden kommt.
