Seltene Ursache eines perinephritischen Abszesses

Im Januar 2021 stellte sich eine 20-jährige Patientin mit seit 3 Wochen bestehenden und nun exazerbierten Beschwerden in der internistischen Notaufnahme unserer Klinik vor. Sie beschrieb eine ausgeprägte Allgemeinzustandsverschlechterung, intermittierendes Erbrechen sowie Diarrhöen und nächtliches Fieber. Eine Infektion mit dem Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) konnte zeitnah ausgeschlossen werden. Zu Beginn der Beschwerden vor 3 Wochen habe die Patientin Zahnschmerzen gehabt, sodass eine ambulante zahnärztliche Vorstellung erfolgte. Dabei zeigte sich ein insgesamt schlechter Zahnstatus mit ausgeprägten Kariesläsionen. Eine Sanierung habe die Patientin bis dato noch nicht durchführen lassen. In den letzten Wochen habe sie schmerzbedingt weniger gegessen und ungewollt 6 kg Gewicht verloren. Auf explizite Nachfrage hin erwähnte sie, rechtsseitige Oberbauch- und Flankenschmerzen gehabt zu haben, sodass eine urologische Mitbeurteilung erfolgte. Urologischerseits lagen keine Vorerkrankungen vor und es bestanden weder in der Notaufnahme noch zuvor dysurische noch zystitische Beschwerden. …
