Innovative Ultraschalldiagnostik bei Nierentumoren

Zusammenfassung

Hintergrund

Nierentumoren werden unter anderem durch die technischen Fortschritte der Ultraschalldiagnostik und durch die flächendeckende Verbreitung von Ultraschallgeräten in Deutschland immer früher erkannt. Die Ultraschalldiagnostik stellt meist das erste bildgebende Verfahren in der Hand des Urologen dar. Bei Nierenläsionen > 1 cm sollte mittels CT/MRT-Diagnostik (Computertomographie/Magnetresonanztomographie) der Fettgehalt gemessen werden, um die Verdachtsdiagnose eines Angiomyolipoms zu bestätigen bzw. das weitere Procedere bei malignomsuspekter Nierenläsion im interdisziplinären Tumorboard unter Einbeziehung sämtlicher Bildgebung festzulegen. Als weiterer Schritt kann besonders bei entzündlichen Läsionen oder Lymphomverdacht eine Biopsie indiziert werden, anderweitige solide Tumoren werden einer tumorstadiengerechten urologischen Therapie zugeführt.

Ergebnisse

Mit der kontrastverstärkten Sonographie kann gegenwärtig nicht sicher zwischen benignen und malignen Nierentumoren unterschieden werden. Dennoch ist die Methode im klinischen Alltag hilfreich zum Ausschluss von „Parenchymzapfen“ und anderen Pseudoläsionen, als nierenschonende, kontrastverstärkte Bildgebung bei entzündlichen Läsionen und für Verlaufskontrollen traumatischer Pseudoläsionen sowie zur Differenzialdiagnose von untypischen Nierenzysten in Abgrenzung zum Nierentumor. Die kontrastverstärkte Sonographie kann ferner die Mikroperfusion von Nierentumoren aufzeigen und so Hinweise für die Unterscheidung zwischen klarzelligen und papillären Nierenzellkarzinomen liefern. Auch für kontrastmittelsonographisch gesteuerte Nierentumorpunktionen ist die Methode optimal geeignet.

Schlussfolgerung

Die kontrastverstärkte Sonographie ergänzt die CT/MRT-Bildgebung durch eine Echtzeitinformation über die Durchblutung des Nierentumors und kann so in der interdisziplinären Nierentumorkonferenz mit der Perfusionsinformation eine mitunter therapieentscheidende Hilfe sein, deshalb sollten die Ultraschalluntersuchungen dort als Video präsentiert und kommentiert werden.
