Nierentransplantation in Harnableitungen

Zusammenfassung

Nierentransplantationen in augmentierte Harnblasen oder Harnableitungen sind selten und machen lediglich 1–2 % aller Nierentransplantationen aus. Meist stellt bei diesen Patienten eine Dysfunktion des unteren Harntraktes die Ursache für das Nierenversagen dar, weshalb eine Korrektur des unteren Harntraktes für einen langfristigen Erhalt der Transplantatfunktion unabdingbar ist. Für gewöhnlich erfolgt die Anlage einer Harnableitung mehrere Monate vor der Transplantation. Neben der Transplantation in das klassische Ileumconduit werden zunehmend auch Transplantationen in kontinente Harnableitungen durchgeführt. Häufigste Komplikationen stellen Bakteriurien und Harnwegsinfekte dar, welche jedoch für gewöhnlich unter entsprechender antibiotischer Therapie nicht zu einem Funktionsverlust der Transplantatniere führen. Die Langzeitergebnisse von Nierentransplantationen in eine Harnableitung sind mit den Transplantationen in eine funktionale originäre Harnblase vergleichbar.
