Over the past decade there has been rapid uptake in the performance of nipple-sparing mastectomy (NSM) for both risk reduction and cancer treatment.
1 – 4 Initially the indications for NSM were fairly stringent, but now are expanding.
4 – 6 Data from observational studies on the efficacy of NSM for risk reduction, acknowledgment that current targeted systemic therapies diminish local as well as distant breast cancer recurrence rates, and growing experience with improved surgical and reconstructive techniques have spurred this change. Increased uptake of genetic panel testing is identifying more high-risk patients as potential candidates for risk-reducing mastectomy, and at the same time more women with breast cancer are seeking bilateral mastectomy, especially those having immediate breast reconstruction.
7 – 10 Increased patient demand for NSM is fueled, at least in part, by the enhanced aesthetics offered by retention of the native nipple-areolar complex (NAC). …
