Ocular flutter is a rare eye movement disorder, most commonly associated with paraneoplastic, auto-immune and para-infectious disorders, but can rarely be seen in metabolic disorders, such as hyperosmolar hyperglycemic state (HHS). Clinicians should be vigilant for HHS in elderly patients with type 2 diabetes, decreased fluid intake, acute illness and concomitant use of drugs that affect glucose metabolism or fluid homeostasis. We report a rare case of ocular flutter and myoclonus in HHS and describe HHS, its risk factors and complications, ocular flutter and other movement disorders seen in hyperglycemia. …
