Eosinophile Ösophagitis

Zusammenfassung

Die eosinophile Ösophagitis ist eine zunehmend häufig diagnostizierte chronisch-entzündliche Erkrankung der Speiseröhre, die in allen Altersgruppen auftreten kann und eine deutliche Prädominanz für das männliche Geschlecht aufweist. Der Häufigkeitsgipfel liegt zwischen dem 30. und 40. Lebensjahr. Ätiologisch sind Nahrungs- und Luftallergene von Bedeutung. Durch eine allergologische Diagnostik kann das verantwortliche Antigen in der Regel nicht identifiziert werden. Leitsymptome bei Erwachsenen sind die Dysphagie und Bolusobstruktion. Bei Kindern ist das Symptomenspektrum variabel und unspezifisch. Typische endoskopische Befunde sind fixierte Ringe/Strikturen, weißes Exsudat, Längsfurchen und Schleimhautödem. Als histologisches Diagnosekriterium gilt eine Eosinophilenzahl von >15 pro „high power field“ (HPF). Zur Behandlung werden topische Kortikosteroide, Protonenpumpeninhibitoren oder empirische Eliminationsdiäten empfohlen. In der Regel ist ein langfristiges Therapiekonzept notwendig. Antiallergika, Immunsuppressiva oder Biologika haben bislang keinen gesicherten Stellenwert.
