Ösophaguskarzinome in der Endoskopie

Ein Überblick der aktuellen Diagnostik und Therapie

Zusammenfassung

Hintergrund

Das Auffinden neoplastischer Läsionen im Ösophagus kann auch für erfahrene Endoskopiker schwierig sein. Eine frühe Diagnosestellung ist jedoch entscheidend für die Möglichkeit einer endoskopischen Therapie mit exzellenter Prognose. Im Gegensatz dazu ist eine Ösophagusresektion mit relativ hoher Letalität und Morbidität assoziiert.

Fragestellung

Zusammenfassung der aktuellen endoskopischen Diagnostik und Therapie bei Ösophaguskarzinomen.

Material und Methode

Der aktuelle Stand der Forschung und Expertenmeinungen wurden analysiert und diskutiert.

Ergebnisse

Die Anwendung moderner High-End-Videoendoskope und virtueller Chromoendoskopie ermöglicht das Auffinden ösophagealer Neoplasien mit hoher Sensitivität. Die anschließende endoskopische Resektion ist sicher und garantiert hohe Remissionsraten bis 94 %. Nach der endoskopischen Therapie der Adenokarzinome sollte zwingend die Ablation der verbliebenen Barrett-Mukosa erfolgen. Bei inoperablen Karzinomen kann im Rahmen der Palliation eine lokale Argon-Plasma-Koagulation, Bougierungstherapie und Stentimplantation eine Symptomlinderung erbringen.
