Okuläre elektrische Stimulation

Therapeutische Anwendung und aktive retinale Implantate bei hereditären Netzhautdegenerationen

Zusammenfassung

Hintergrund

Elektrische Stimulation am Auge hat eine lange Geschichte. Unterschwellig appliziert kann elektrischer Strom therapeutische Wirkungen bei hereditären Netzhauterkrankungen entfalten, überschwellig eingesetzt vermag er Seheindrücke auszulösen. Mit retinalen Multielektrodenfeldern lassen sich bei überschwelliger Stimulation verwertbare Bildeindrücke bei Blinden erzeugen.

Ziel der Arbeit

Diese Arbeit soll einen Überblick über den aktuellen Stand und die derzeit laufenden Studien zur therapeutischen Elektrostimulation liefern. Darüber hinaus sollen die Herausforderungen, chirurgische Konzepte und die Ergebnisse der visuellen Rehabilitation durch aktive retinale Implantate besprochen werden.

Material und Methoden

Es wird eine Übersicht zu den publizierten Ergebnissen klinischer Studien zur transkornealen Elektrostimulation und elektronischen Netzhautimplantaten gegeben, unter besonderer Berücksichtigung der klinischen Anwendbarkeit.

Ergebnisse

Schon erste Studien zur therapeutischen transkornealen Elektrostimulation verliefen sehr Erfolg versprechend. Die bisher größte kontrollierte Studie bei Patienten mit Retinitis pigmentosa (RP) hat viele positive Trends und einige signifikante Ergebnisse in elektrophysiologischen Auswertungen hervorgebracht. Derzeit sind 2 zugelassene aktive retinale Implantate auf dem Markt, das Argus II Retinal Prosthesis System® von SecondSight® und das Alpha-IMS© von Retina Implant AG. Mit beiden Implantaten lässt sich ein Sehvermögen wieder herstellen, die maximale Sehschärfe liegt bei 0,02 bis 0,04 respektive.

Diskussion

Geplante weitere Analysen und Anschlussstudien an großen Patientengruppen sollen weiteren Aufschluss liefern, inwieweit die elektrische Stimulation bei RP-Patienten sinnhaft eingesetzt werden kann. Die Herausforderungen derzeitiger aktiver retinaler Implantate liegen in der technischen Haltbarkeit und der Steigerung der räumlichen Auflösung.
