Zusammenfassung

Der demographische Wandel der Gesellschaft führt einerseits zu einer Zunahme der Häufigkeit von Aortenstenosen und Mitralinsuffizienzen, andererseits bringt das höhere Lebensalter der Patienten eine erhöhte Multimorbidität und damit eine Steigerung des Operationsrisikos mit sich. Neben dem bisherigen Goldstandard der chirurgischen Therapie haben sich zunehmend auch minimal-invasive, kathetergestützte Interventionen etabliert. Für die differenzierte Indikationsstellung und den Therapieerfolg spielt ein multidisziplinäres Herzteam eine wichtige Rolle. Weiterentwicklungen auf dem Gebiet der kathetergestützten Aortenklappenimplantation (TAVI) werden neben einer zunehmenden Mortalitätssenkung zu besseren Ergebnissen und tendenziell zur Ausweitung der Indikationsstellung führen. In der minimal-invasiven Mitralklappentherapie werden neue innovative Methoden geprüft, die zukünftig neue Therapieoptionen erschließen könnten.
