Zusammenfassung

Die Hüftkopfnekrose ist eine avaskuläre Nekrose des Femurkopfes multifaktorieller Genese. Eine konservative Behandlung als therapeutische Option bietet sich nur im Frühstadium an. Diese ist aber rein symptomatisch und nicht kausal. Da die meist männlichen Patienten in der Regel mitten im Erwerbsleben stehen, sollte die Implantation eines künstlichen Hüftgelenks möglichst lange hinausgezögert werden. Es bieten sicher daher in den Stadien ARCO I–III verschiedene gelenkerhaltende operative Verfahren an. Neben der reinen Anbohrung bieten die Umstellungsosteotomie und die Fibulagrafts probate Alternativen, deren Vor- und Nachteile abgewogen werden müssen. Neuere Methoden wie additive Stammzellbehandlungen und Verwendungen von „platelet rich plasma“ (PRP) in Kombination mit der Anbohrung müssen ihren Effekt noch nachweisen.
