Operative Therapie des Endometriumkarzinoms

Zusammenfassung

Die optimale Behandlung des Endometriumkarzinoms ist die Operation. Sie ist sowohl diagnostisch als auch therapeutisch die Methode der Wahl. Die histologische Aufarbeitung gibt Aufschluss über weitere adjuvante Therapieoptionen und die Prognose. Das bis 1988 empfohlene rein klinische Staging hat sich als nicht ausreichend reliabel erwiesen. Zusätzlich zur Laparotomie mit Hysterektomie und Adnexektomie wird eine systematische pelvine und paraaortale Lymphadenektomie empfohlen. Im Stadium pT1a bzw. pT1b G1–G2 ist die Lymphadenektomie fakultativ. An spezialisierten Zentren wird in ausgewählten Fällen ein laparoskopisch assistiertes vaginales Vorgehen gewählt und von vergleichbarem onkologischem Outcome berichtet. Die primäre Bestrahlung scheint der Operation unterlegen und sollte nur bei inoperablen Patientinnen zur Anwendung kommen. Laufende Studien werden zeigen, ob das Sentinelverfahren auch beim Endometriumkarzinom zuverlässige Ergebnisse zeigt und eine Lymphadenektomie bei tumorfreiem Sentinellymphknoten vermieden werden kann.
