Zusammenfassung

Vesikovaginale Fisteln stellen ein seltenes Problem in der westlichen Welt dar, sind jedoch häufig in Entwicklungsländern anzutreffen. In Deutschland ist die häufigste Ursache die Hysterektomie. Vesikovaginale Fisteln können je nach Fistel- und Patientencharakteristika transvaginal oder transabdominell behandelt werden. Die Inzidenz und Komplexizität von urorektalen Fisteln nimmt mit der Zunahme der Therapiesequenzen des Prostatakarzinoms zu. Insgesamt gibt es keinen eindeutigen Konsensus über den optimalen chirurgischen Zugangsweg. Die chirurgische Therapie sowohl von vesikovaginalen als auch von urorektalen Fisteln geht mit einer hohen, dauerhaften Fistelverschlussrate einher. Bei beiden Entitäten kann, wenn die Fistel früh detektiert wird, je nach zugrunde liegender Ursache, auch konservativ mittels vorübergehender Harnableitung behandelt werden. Für beide Krankheitsbilder gilt, dass der Fistel-Repair bei bestrahlten Patienten deutlich geringere Erfolgsraten aufweist. Auch ist ein spontaner Fistelverschluss bei radiogenen Fisteln nicht zu erwarten.
