Opioidinduzierte Immunsuppression

Ein klinisch relevantes Problem?

Zusammenfassung

Zahlreiche In-vitro-Studien und Untersuchungen im Tiermodell deuten darauf hin, dass Opioide möglicherweise immunsupprimierend wirken und das Infektionsrisiko steigern können. Die klinische Relevanz dieser Befunde ist unklar. Ziel dieser Übersichtsarbeit war es daher, die relevanten tierexperimentellen und humanen Studien zu retroviralen Infektionen (i.e. „human immunodeficiency virus“, HIV), Sepsis und Pneumonie, postoperativer Schmerztherapie und Therapie chronischer Schmerzen zusammenzufassen und zu bewerten. Während die Mehrzahl der tierexperimentellen Studien ein erhöhtes Infektionsrisiko unter Opioidtherapie zeigt, sind die Studienergebnisse beim Menschen widersprüchlich. Die klinischen Studien wurden vielfach an kleinen Patientenkollektiven durchgeführt und entsprechen häufig nicht den Anforderungen der evidenzbasierten Medizin. Ein Zusammenhang zwischen der Gabe von Opioiden und einem gesteigerten Infektionsrisiko ist beim Menschen somit bisher nicht überzeugend belegt, kann aber auch nicht sicher ausgeschlossen werden.
