Optimierung der Prothesenpositionierung und Balancierung in der Knieteilprothetik mithilfe bildgestützter Roboterassistenz

Zusammenfassung

Hintergrund

Computerassistierte Operationsverfahren wie die Navigation oder patientenspezifische Instrumentierung (PSI) haben sich bisher in der Endoprothetik des Kniegelenkes nicht flächendeckend durchgesetzt. Trotz des evidenten Vorteils für die Präzision der Prothesenpositionierung überwiegen die Gegenargumente in Bezug auf Aufwand und Kosten bzw. wird der klinische Nutzen hinterfragt. Allerdings ist gerade die Teilprothetik ein interessantes Einsatzgebiet für technische Operationsassistenz, da der Zusammenhang von Erfahrung des Operateurs und sauberer Operationsdurchführung und dem Outcome und Survival der Prothese eindeutig nachgewiesen ist.

Fragestellung

Die vorliegende Arbeit beschreibt den dezidierten Workflow aktueller bildgestützter Robotertechnologien und gibt einen Überblick über mögliche Vorteile zum händischen Standardverfahren.

Material und Methoden

Der Übersichtsartikel fasst die aktuell verfügbare Literatur zur Thematik zusammen und gibt Einblicke in die klinische Praxis der Autoren.

Ergebnisse und Diskussion

Die bildgestützte dreidimensionale Vorplanung der Operation anhand des CT-Modells bietet zahlreiche Zusatzinformationen über das Kniegelenk, die die korrekte Größenwahl und anatomische Ausrichtung des Implantats verbessern. Durch zusätzliche intraoperative Parameter wie die Ganzbeinachse, das Laufprofil der Prothese und die Stabilität können Feinjustierungen zur Optimierung der Gelenkbalancierung vorgenommen werden. Die Robotertechnik hilft dann bei der präzisen Umsetzung des Planes. Diese Möglichkeiten scheinen in einer signifikanten Reduktion der chirurgischen Fehler und konsekutiv auch des Revisionsrisikos der Teilprothesen zu münden. Damit hat die Technologie das Potenzial, die Versorgungsrealität zugunsten eines höheren Anteils an Teilprothetik zu verändern.
