Entwicklung und Validierung des Stapesplasty Outcome Test 25 (SPOT-25)

Zusammenfassung

Hintergrund

Die Otosklerose führt zu Einschränkungen der gesundheitsbezogenen Lebensqualität (HRQOL). Bisherige Untersuchungen basieren jedoch ausschließlich auf tonaudiometrischen Daten sowie nicht validierten krankheitsunspezfischen HRQOL-Messinstrumenten.

Zielstellung

Ziel der Studie war es, das erste validierte krankheitsspezifische Messinstrument zur Bestimmung der Lebensqualität bei Patienten mit Otosklerose zu entwickeln.

Material und Methoden

Nach einer statistisch begründeten Itemreduktion des Stapesplasty Outcome Test 47 (SPOT-47) auf 25 Items erfolgte eine Validierung des resultierenden SPOT-25 prospektiv anhand von 52 Patienten, welche sich einer Stapesplastik unterzogen. Zusätzlich zu einem Gesamtscore wurden die Subscores Hörfunktion, Tinnitus, psychisches Befinden und soziale Einschränkung konzipiert.

Ergebnisse

Der SPOT-25 hat eine hohe interne Konsistenz (Cronbach-α > 0,7), kann suffizient zwischen Otosklerosepatienten sowie ohrgesunden Probanden diskriminieren und weist eine gute Test-Retest-Reliabilität (r = 0,85) auf. Postoperativ zeigte sich eine signifikante Verbesserung der HRQOL bei hoher Responsivität.

Schlussfolgerung

Mit dem SPOT-25 steht nun das erste validierte krankheitsspezifische Messinstrument zur Bestimmung der HRQOL bei Otosklerosepatienten zur Verfügung, welches zur Qualitätskontrolle im klinischen Alltag eingesetzt werden kann.
