16. Palliative chirurgische Verfahren des Magen- und Kardiakarzinoms

Zusammenfassung

Während die komplette chirurgische R0-Resektion, oft im Rahmen multimodaler Therapiekonzepte, für Patienten mit Magenkarzinom die einzige Chance auf Kuration bietet, werden etwa 65 % aller an einem Magenkarzinom leidenden Patienten erst in einem lokal (zu) fortgeschrittenem und/oder metastasiertem Stadium diagnostiziert (Jemal et al. 2011, 2010, Ochenduszko et al. 2015). Palliative (chirurgische) Verfahren sind somit für gut zwei Drittel aller Patienten mit Magenkarzinom von besonderer Bedeutung.

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
