Zusammenfassung

Nach 5 Jahren leben nur noch 8–9 % der Patienten mit einem duktalen Adenokarzinom des Pankreas („pancreatic ductal adenocarcinoma“ [PDAC]). Im Jahr 2030 könnte das PDAC die zweithäufigste krebsbedingte Todesursache sein. Hauptgründe sind eine meist metastasierte Erkrankung bei Diagnosestellung, die aggressiven tumorbiologischen Eigenschaften und die ausgeprägte Therapieresistenz. Die Diagnose beruht vor allem auf der Computertomographie mit anschließender Diagnosesicherung mittels Biopsie oder Operation. Eine Kombinationschemotherapie wie FOLFIRINOX (5-Fluorouracil [5-FU], Folinsäure, Irinotecan, Oxaliplatin) oder Gemcitabin/nab-Paclitaxel (an Albuminnanopartikel gebunden) in der Erstlinie sowie nanoliposomales Irinotecan/5-FU in der Zweitlinie kann ein längeres Überleben erreichen. Aufgrund des zunehmenden molekularen Verständnisses sind präzisionsmedizinische Ansätze im aktuellen Fokus. Dieser Beitrag soll einen Überblick über Entstehung, Diagnostik und Therapie des PDAC geben.
