Zusammenfassung

Die chronische patellofemorale Instabilität ist ein multifaktorielles und häufig angeborenes Krankheitsbild. Die Luxation der Patella erfolgt hierbei meist atraumatisch. Eine rein konservative Therapie ist zumeist frustran. Die operative Therapie erfordert die Berücksichtigung aller pathologischen Veränderungen der anatomischen Faktoren, die die Patella stabilisieren. Neben dem nahezu pathognomonisch rupturierten medialen patellofemoralen Ligament (MPFL) spielen die frontale Beinachse, die Rotation von Femur und Tibia, die Form der Trochlea sowie der Abstand der Tuberositas tibiae zum Sulcus trochlearis und die Höhe der Patella eine entscheidende Rolle. Häufig muss zusätzlich zu einer weichteiligen Stabilisierung auch das knöcherne Alignment korrigiert werden. Nur die Berücksichtigung aller Risikofaktoren sowohl beim Erwachsenen als auch beim jugendlichen Patienten kann eine Rezidivinstabilität vermeiden.
