Neue Therapieoption im klinischen Alltag

Zusammenfassung

PCSK9 (Proproteinkonvertase Subtilisin/Kexin Typ 9) bindet an den LDL („low-density lipoprotein“)-Rezeptor und führt in Leberzellen zu seinem Abbau. Durch Hemmung von PCSK9 kommt es zu einer Erhöhung von LDL-Rezeptoren und damit zur Senkung von LDL-Cholesterin im Blut. In Deutschland stehen zurzeit zwei Antikörper gegen PCSK9 für die klinische Therapie zur Verfügung, dies sind Evolocumab (Repatha®) und Alirocumab (Praluent®). Die klinischen Studien haben gezeigt, dass die Therapie mit diesen Antikörpern, die alle 2 oder 4 Wochen subkutan mit einer Stechhilfe selbst injiziert werden müssen, die LDL-Cholesterin-Spiegel im Blut zusätzlich zu einer bereits bestehenden Lipidtherapie im Mittel um 50–60 % senken können. Die Daten der bisherigen Studienprogramme zeigen, dass die Therapie sicher ist, wobei Langzeitdaten noch fehlen. Ergebnisse der großen, bereits laufenden kardiovaskulären Endpunktstudien stehen aus, wobei erste Analysen einen günstigen Effekt erwarten lassen. Durch diesen neuen effektiven Therapieansatz eröffnet sich eine neue Perspektive für die Behandlung von Patienten, deren LDL-Cholesterin-Werte trotz bisheriger maximaler lipidsenkender Therapie noch über dem entsprechenden Zielwert liegen und die an einer vorbestehenden kardiovaskulären Erkrankung, einer Statinintoleranz oder einer genetisch bedingten familiären Hypercholesterinämie leiden, oder für Betroffene, die sich bereits regelmäßig einer LDL-Apharese unterziehen müssen.
