22. Peniskarzinom

Zusammenfassung

Das Peniskarzinom tritt am häufigsten in einem Lebensalter von 55–65 Jahren auf; die Inzidenz unterscheidet sich in verschiedenen Ländern deutlich und liegt besonders niedrig in arabischen Ländern. Risikofaktoren sind Phimose und Balanitis (bei im Säuglingsalter beschnittenen Männern gibt es praktisch kein Peniskarzinom), Genitalhygiene, sexuelle Aktivität wegen des Zusammenhangs mit einer HPV-Infektion sowie Nikotinabusus. Unter einem Peniskarzinom werden Plattenepithelkarzinome, Nicht-Plattenepithelkarzinome (z. B. Basalzellkarzinom, Morbus Paget, Melanom) sowie prämaligne kutane Läsionen (z. B. Cornu cutaneum) zusammengefasst. Bei der Diagnostik ist neben der Biopsie zur Therapieauswahl v. a. ein Lymphknotenstaging wichtig. Zur Therapie sind je nach Stadium eine organerhaltende Resektion (plus Radiatio) oder Penektomie mit/ohne Lymphadenektomie und zusätzlich ggf. eine kombinierte Chemotherapie indiziert.
