Konventionelles vs. molekulares Grading

Zusammenfassung

Der histologische Malignitätsgrad ist neben Tumorstadium und Tumortyp beim Peniskarzinom prognostisch und therapeutisch sehr relevant. Bei abgestuften Therapiekonzepten hat seine Bedeutung in den letzten Jahren deutlich zugenommen. Graduiert werden hauptsächlich Tumordifferenzierung und Zellproliferation sowie bei einigen Tumorgruppen auch Nekroseneigung, Invasionsform und Metastasierungsneigung. Die Ausprägung dieser unterschiedlichen Tumoreigenschaften ist in Abhängigkeit vom Tumortyp anhand Hämatoxylin-Eosin- (HE-)gefärbter histologischer Schnittpräparate fassbar. Ergänzende immunhistochemische und molekularpathologische Untersuchungen können zu einer höheren Prognosegenauigkeit beitragen. Zur Objektivierung des subjektiven konventionellen Grading bieten sich quantitative Analysen an, für die sich besonders die Zellproliferation eignet. Trotz interessanter Entwicklungen bedarf es weiterer Untersuchungen für den breiten Einsatz im klinischen Alltag.
