Perspektiven der CAR-T-Zell-Therapie

Zusammenfassung

Aufgrund der großen Anzahl möglicher Modifikationen und der bisherigen Erfolge gehören CAR‑T (T-Zellen mit einem chimären Antigenrezeptor) zu den innovativsten Medikamenten in der Onkologie. Im August 2018 wurden erstmals in Europa zwei Produkte zugelassen, für die Behandlung von vorbehandelten aggressiven Lymphomen, eines davon auch zur Behandlung der akuten lymphatischen Leukämie bei Kindern und jungen Erwachsenen bis 25 Jahre. In absehbarer Zeit werden weitere Zulassungen folgen, so z. B. beim Mantelzelllymphom und beim multiplen Myelom. Klinische Studien mit neuen Konstrukten werden bei fast allen onkologischen Erkrankungen durchgeführt. Die Einführung dieses völlig neuartigen Prinzips führt zu besonderen Herausforderungen: (1) Die Gabe von gentechnisch veränderten „lebenden Medikamenten“ ist regulatorisch kompliziert und erfordert vermutlich eine lebenslange Überwachung der Patienten, (2) Hersteller und Behörden stellen hohe Anforderungen bei der Durchführung an einzelne Zentren, (3) bei einem Preis von ca. 280.000 € bei den beiden derzeit zugelassenen Produkten müssen sozioökonomische und ethische Fragen beantwortet werden. Trotzdem werden CAR‑T die therapeutische Landschaft in der Onkologie verändern.
