Netz- und matrixgestützte Implantatrekonstruktion

Zusammenfassung

In den vergangenen Jahrzehnten hat sich die Technik der rekonstruktiven implantatbasierten Brustchirurgie durch die onkologischen Innovationen der haut- (SSM) und nippelsparenden Mastektomie (NSM) in Kombination mit neuen Materialien wie synthetischen Netzen (SN) und azellulären dermalen Matrizes (ADM) stetig weiterentwickelt. Ziel ist die sofortige komplette Abdeckung und Stabilisierung der gesamten Implantatloge, kranial durch den mobilisierten M. pectoralis major, kaudal durch das am Muskel fixierte Netz oder ADM mit gleichzeitiger Stabilisierung des Implantats. So kann reproduzierbar und zuverlässig eine deutlich bessere Ästhetik erreicht werden als mit Standardtechniken ohne Netz oder ADM. Darüber hinaus können andere Muskelpräparationen des M. serratus anterior oder sogar des M. latissimus dorsi zur Implantatabdeckung vermieden werden. Die Operationstechniken der Netz- oder ADM-unterstützten Implantatrekonstruktion sind anspruchsvoll, mit einer merklichen Lernkurve verbunden und nicht standardisiert. Es gibt immer mehr unterschiedliche Medizinprodukte an synthetischen Netzen und biologischen Matrizes, deren klinische Einführung oft ohne nennenswerte vorausgegangene klinische Studien erfolgt. Das Implantat- und Netzregister der Arbeitsgemeinschaft für ästhetische, plastische und rekonstruktive Operationsverfahren in der Gynäkologie (AWOgyn), in dem inzwischen über 110 Brustoperateure ihre Fälle dokumentieren, ermöglicht eine kontinuierliche Einordnung der verwendeten Materialien. Der differenzierte Einsatz der bereits in der klinischen Routine verwendeten oder auch neuer Netz- und ADM-Materialien ist ein Feld der aktuellen fachlichen Diskussion, des Erfahrungsaustausches und klinischer Forschung.
