Medizinethische Entscheidungen am Lebensende bei chronischen Erkrankungen

Zusammenfassung

Entscheidungen am Lebensende sind eine große ärztliche Herausforderung. Das ärztliche Handeln muss an den 4 ethischen Prinzipien „Wohlergehen des Patienten, keinen Schaden zufügen, Prinzip der Autonomie des Patienten und Gerechtigkeit“ ausgerichtet sein. Primäre Grundlage ärztlichen Handelns ist die medizinische Indikation, danach folgt die Berücksichtigung des (mutmaßlichen) Willens des Patienten. Der Arzt ist zur Lebenserhaltung innerhalb des Patientenwillens verpflichtet. Korrekt ausgefüllte Patientenverfügungen entbinden den Arzt nicht von der Verpflichtung, die aktuelle Gesundheitssituation und deren Behandlungsmöglichkeiten mit dem Willen des Patienten in der schriftlich niedergelegten Patientenverfügung abzugleichen. „Advance care planning“ ist eine vorausschauende Gesundheitsplanung, die einen andauernden Kommunikationsprozess zwischen Patienten, ihren gesetzlichen Vertretern und den Behandelnden und Betreuenden beschreibt. Ziel ist, mögliche künftige Behandlungsentscheidungen auch für den Fall, dass die Betroffenen selbst nicht entscheiden können, vorauszuplanen.
